Academic Forms
The following academic forms are available in the Registrar's Office and may be submitted to the Registrar's Office from 8:00 a.m. to 4:00 p.m. as noted below:
Add/Drop - the last day to add a class is the 5th day of classes and the last day to drop a class without transcript notation is the 15th day of classes. Schedule changes are free for the first five days of classes. A fee of $5.00 for each drop is charged after the first five days of classes.
CR/D/F - sophomores, juniors, and seniors may declare a class CR/D/F. The CR/D/F form must be submitted before the end of the 5th day of classes.
Audit - the last day to declare a class as an Audit is the 5th day of classes. Students will receive the grade of AU on their transcript.
Independent Study - students enrolled for an independent study must submit the independent study form by the 5th day of classes.
Writing Intensive Option - students enrolled for a course listed as Writing Intensive Option who want WIO credit must submit the WIO form by the 5th day of classes.
Course Overload - forms, for students enrolled for more than 19 hours, must be submitted by the 5th day of classes.
Withdrawal - students who want to withdraw from a course must submit the withdrawal form after the drop period and before the 45th day of classes (see academic calendar for exact dates). Students will receive the grade of WP or WF on their transcript.
Medical Withdrawal -students must schedule an appointment with the Dean of Student Life to request a Medical Withdrawal. If approved, students will receive the grade of W on their transcript. Medical Withdrawal Forms must be submitted after the end of the withdrawal period.
Declaration of Major/Minor - students may declare a major or minor as early as the second semester of their freshmen year. All students should have declared a major by the first semester of their junior year.
Self-Designed Major/Minor - same as Declaration of Major/Minor above.
Change of Advisor - form must be submitted when student wants to change advisors.
Course Approval for Transfer Credit - form should be submitted prior to enrolling for the transfer course to assure that the class would transfer to Westminster.
